processes my personal data in the following extent: name, surname, telephone number, email for the purpose of contacting me and the related services of the Controller in accordance with Article 6, 1(b) of the Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of natural persons with regard to the processing of personal data and on the free movement of such data, and repealing Directive 95/46/EC (General Data Protection Regulation, hereinafter referred to as “GDPR”).
I am aware of and consent to the fact that the Controller may provide the personal data I have supplied (to the extent mentioned above) for potential further processing of personal data to other processors who process personal data for the Controller and provide guarantees of appropriate and proper personal data processing and adequate protection of personal data, including their protection through appropriate technical or organizational measures against unauthorized or unlawful processing and against accidental loss, destruction, or damage.
The personal data provided to the Controller will be processed for the duration of the contractual relationship with the Controller and subsequently for the necessary retention period in accordance with applicable legal regulations of the Czech Republic.
Furthermore, I hereby declare that I am aware of my right to information about the processing of my personal data (particularly information about the category of processed personal data, the purpose, method, and duration of processing, and potential recipients of personal data), the right to request access to my personal data from the Controller, their correction, or deletion (especially in the case of incorrect, incomplete, or outdated personal data), and the right to file a complaint with the supervisory authority. I understand that from May 25, 2018, I also have the right to the portability of my personal data to a new data controller and the right to restrict processing.
I further declare that the personal data I have provided is true, I have provided it completely freely, knowingly, voluntarily, and I am aware that providing this data is necessary for the realization of the above-mentioned service.
